Forms for Complete Wellness Clinic

For your convenience we have provided our intake forms below. Before your first appointment, please print and fill out the appropriate forms and bring it with you to your appointment. If you are unable to do so, please arrive 10 minutes prior to your scheduled appointment time to complete them. For naturopathic and counseling appointments, please allow 20 minutes prior to your appointment time to complete the intake forms in person.

Important

Please acknowledge that our office policy requires twenty four (24) hours notice of cancellation. Without 24 hours prior notification, you will be billed a cancellation fee. For counseling appointment cancellations, 48 hours notice is required. All NSF cheques will also be charged a fee.
